What Information Should You Know?

Household Income

  • Father's Annual Gross Income
  • Mother's Annual Gross Income
  • Spouse's Annual Gross Income (if married)
  • Legal Guardian's Annual Gross Income (if applicable)

Financial Contributions

If the family receives support from an outside source, specify:

  • Name of the contributor
  • Amount given
  • Purpose of the contribution

If your family owns agricultural land, provide the total hectares.

Print the form on A4 paper, have them fully signed by the proper authorities, and ensure all required information is complete. If using alternative documents, include all necessary details.

Save scanned or photographed forms and upload them to the correct fields in the E-Application System. Use the preview to ensure files are complete, correct, and viewable to avoid disqualification.
